Attestation of Death Documents

NOC to carry Mortal Remains or Ashes of the Deceased and Attestation of Death Documents 

Death related documents for carrying mortal remains and ashes of the deceased to India will be processed as per the convenience of the applicant irrespective of consular jurisdiction.

 How to Apply in Person:

1.

Download the Application for Miscellaneous Services , fill it up carefully and affix your photograph. 

Here, the “applicant” is a person who is a relative/friend of the deceased or representative of a funeral home, authorized for processing the documents.

2.

Submit current passport of the deceased in original and a copy.

3.

Submit current valid passport of the applicant in original and a copy. If the documentation is done through funeral home, the representative may submit his/her photo I/card and duly-signed authorization letter issued by the funeral home.

4.

Submit original and photocopy of the document showing valid US Visa Status of the applicant (relative or friend of the deceased who is doing the documentation work).

5.

Submit original and photocopy of any of the following documents as proof of residence of the applicant (relative/friend of the deceased): US Driving License, State Identity Card, Lease Agreement, Mortgage Deed or Utility Bills (Electricity, water or gas bills).

6.

Carry all original supporting documents for presentation to the Consular Officer. Photocopies of the documents may be attached with the application. 

7.

To carry Mortal Remains: Please submit original and one copy of the following: (a) Death Certificate (clearly stating cause of death), (b) Embalming Certificate from the concerned local authority, (c) Certificate from the concerned local authority stating that body of the deceased is free from any kind of infection,  and (d) Burial/Transit permit. Applicant may also ascertain from the concerned airlines about additional documentary requirements, if any, and obtain those from the mortuary/funeral home.

To carry Ashes: Please submit original and one copy of the following: (a) Death Certificate and (b) Cremation Certificate. Applicant may also ascertain from the concerned Airlines about additional documentary requirements, if any and obtain those from the mortuary/funeral home. 

9.

Fee: 

NOC for carrying mortal remains: If the deceased held Indian passport - Gratis (No Fee); and if the deceased held non-Indian passport - US$ 60 (Sixty only) cashier cheque or money order in favor of Consulate General of India, Atlanta. And a separate cashier cheque or money order for  US$2(Two) in favor of Indian Community Welfare Fund.

NOC for carrying ashes: If the deceased held Indian passport – Gratis (No fee); and if the deceased non-Indian passport – US$ 40 (Forty only) cashier cheque or money order in favor of Consulate General of India, Atlanta. And a separate cashier cheque or money order for US$2(Two) in favor of Indian Community Welfare Fund.

For attestation of Death certificate in respect of Indian as well as non-Indian passport holders: US$10.00(Ten Only) for attestation of each certificate. Cashier cheque or money order in favor of Consulate General of India, Atlanta. And a separate cashier cheque or money order for US$2(Two) in favor of Indian Community Welfare Fund.  

The fees may also be paid in cash.

Timings for Submission of Applications: 10.00 AM to 12.30 PM on all working days.  No prior appointment is required.

Processing Time:  Delivered on the same day from 3.00 pm to 4.00 pm.

 How to Apply by Mail: 

 

  1. 1. Completed "Misc. Services" application form with one passport size photo affixed in the box.
  2. 2. Fill in all the information correctly. Incomplete application will be returned without service.
  3. 3. Please attach the Clear Notarised photocopy of the  Passport, Address Proof, Legal Status. 
  4. 4. Copy of valid Visa/Green Card.
  5. 5. Copy of Proof of address (utility bill, driver license or lease agreement).
  6. 6. If the applicant is holding a B1/B2 Visa
    • He/She should provide his/her son/daughter/relative's current passport copy.
    • Son/daughter/relative's driver's license copy.
    • A letter from him/her to the Consular officer requesting this service for his/her   parents/relative.
  1. 7. Fee is to be paid by cashier’s check or money order.
  2. 8. Personal checks, Debit cards and Credit cards are not accepted.
  3. 9. If application is being sent by post please sent a separate $20/- cashier’s check or money order fee for return mail. Or Enclose a trackable pre-paid self-addressed return mailing envelope.

 

 

 

Processing time: Five to seven working days from the date of receipt of application, subject to submission of all requisite documents.
