 The Consulate reserves the right to reject attestation of documents the contents of which are objectionable or contrary to the Rules.

 The Consulate does not attest any document issued by authorities in India. If any document issued by authorities in India (such as birth and death certificates, affidavits, diplomas, marriage certificate and educational certificates issued by authorities in India ) are required to be presented to authorities in USA, these should be apostilled in India.  Birth Certificate is issued on the basis of entries in Indian passport.

 

 Supporting Documents: Photocopy of all supporting documents should be notarized and attached to the application. Do not send any original documents.

How to Apply:

 Download the Miscellaneous application Form bullet, print & sign it paste the photograph and Post it to the Consulate along with the other supporting documents. If application is being sent by post please send a separate $20/- cashier’s check or money order fee for return mail. Or Enclose a track-able prepaid self-addressed return mailing envelope.

 

(Supporting Documents)

Please attach the Clear Notarised photocopy of the  Passport both Indian and US, Address Proof, Legal Status. 

 List of documents valid as a proof of Address/Residence (Copy of any one of following):

  • US Driving License OR
  • Utility Bill OR
  • Lease Agreement OR
  • State Identity Card OR
  • Mortgage Deed

 Proof of US Visa Status (Copy of any one of the following):

  • Visa page of Passport (i.e. H1B, H4, L1, L2 etc.) OR
  • Green Card OR
  • Employment Authorization Document (Work Permit) OR

I-797, I-140 or I-20 (If approval is pending, attach a handwritten note detailing the efforts being taken to regularize status)

 

 

Fee:  A Fee of $ 25 (Twenty Five) in favor of Consulate General of India. Atlanta and an additional  fee of $2 (Two ) per application with a separate cheque or money order in favor of Indian Community Welfare Fund.

 

(Personal Cheques, Credit Cards, Debit Cards Or Other Banking Instruments Are Not Accepted)

 

Processing time: 5-7 working days from the date of receipt of applications by post. This is subject to submission of all requisite documents by the applicant(s).

Incomplete applications will be sent back without the service indicating the requisites to complete.
